How to format CSV file for import into PayPal MultiOrder Shipping

Symbols in any of the fields cannot be used. I was able to place dashes (-) and periods (.).
- Example: "Jane.John", "Doe",
If the fields are meant to be blank, then delete the placeholders if viewing in a text editor.
State is the 2 character abbreviation. Here is a list of state abbreviations. 50States or stateabbreviations.us.
Country would be "US" for United States.
The Phone Number can't have parenthesis in it. Separate digits as follows "000-000-0000".
I was informed by the system that Shipping Carrier is always "USPS".

Download this version if you are going to edit in a text editor, such as Notepad++. (This version has placeholders so that if you double-click the 'field' within the parenthesis, you can just replace with text.)
Tip(s) for Notepad++:
-- If you double-click the 'field' within parenthesis and press Ctrl-H, it will bring up the "replace" options and you can replace the items you don't need with no characters (press Delete in the 'replace with' option).
Download this version if you are going to edit in Excel or in your choice of spreadsheet editor. (This has no placeholders as it will be quite annoying if you had to keep deleting cells in the spreadsheet :) )
